Memory Care

Memory care is specialized care for people living with Alzheimer’s, Dementia, and other forms of memory loss. Boulder Post Acute provides a safe and secure environment where staff can closely monitor the health of each resident. Programs, activities, and events are designed to help our memory care residents work on their cognitive abilities, while also getting to enjoy passions and hobbies.

24-Hour Skilled Nursing

A stay at Boulder Post Acute begins with an evaluation by our team of physicians, therapists, nurses, social services, and other support staff. These meetings allow us to design a personalized care plan focused on returning you to your highest level of independence as quickly as possible.

Long-Term Care

Our long-term care services were designed to meet the daily needs of those who have serious, ongoing health conditions or disabilities. Our goal is to help patients live as independently and safely as possible while providing assistance with activities of daily living such as bathing, dressing, personal hygiene, and nutrition.

Behavioral Health

Our behavioral health program provides a secure unit that focuses on the prevention and treatment of behaviors that affect the emotional, social, and psychological well-being of our patients. We get to know how our patients act, interact, and react, so we can provide quality healthcare that helps them deal with or overcome certain behaviors. We partner with Rocky Mountain Behavioral Health Solutions to provide the best care for those with behavioral health needs.
